From the Navigation Bar, select Forms iQ

Within the Practice Form tab, select New Form

A New Lead Form with required fields will load

You can Add and Remove Fields to customize your form

Add a Form Name

You also have the option to add a Form Description

When you select Save and Preview, you can view the form

Select Submit Lead to save your changes
