This guide will demonstrate how to use the different types of fields in the Form Builder
TextBox: Allows users and clients to write freely
Number: A text box that only allows numeric characters
Date: By naming the heading, the Date field allows you to keep track of things such as events and signatures
AutoComplete: Creates a searchable text box with pre-written answers added from the Option
AutoComplete - Long: Creates a multi-line dropdown combo for longer text entries. Shows searchable text box with pre-written answers added from the Option.
By default, this field displays up to 4 lines of text. If the input extends beyond 4 lines, a scroll bar will appear, starting from the 5th line.
Note: You can resize the 'Autocomplete - Long' field by dragging the adjustable corner located at the bottom right.
Drop Down: Creates a clickable menu with specific choices
Switch: Creates a “toggle” or a switch
Checkboxes: Allows users and clients to select multiple choices
Radio Button: Allows users and clients to select only one choice
Signature: An electronic signature. Changing the heading allows you to dictate who should sign where
Rating: Allows users to create named rows with columns of boxes for selection
Array: Helpful in formatting, the Array field allows users to separate sections of fields, and make these sections repeatable with the Add More Button
Title: A read-only text feature that helps you format your form by creating a title for each section
Content: A read-only block of text
You can expand this box as well by pressing the expand button. Click anywhere outside the box to minimize it and save your work
Image: Allows you to insert an image into your form
IMPORTANT: Image must be uploaded in PNG or JPG Format, and the size should be less than 2 MB and 1000x1000p
Link: Creates a hyperlink from the link you choose
Page Break: Allows you to put a break anywhere in a form and creates a new page or a split between pages
Staff: Denotes a drop-down box to select a staff member. Select at least one Title to filter your staff
Client: Creates a searchable drop-down box with all active clients. It can auto-populate other boxes with client demographics.
By adding the field Client and then selecting the field Name, Email, Address, or Phone the fields will be automatically filled in based on the client selected in the Client field
It is also possible to add the field types Name, Email, Address, or Phone by their own.
Start and End Time: Allows you to register the start time, duration, and end time of the sessions
Diagnosis Code: Allows you to select a Diagnosis Code from the dropdown. The dropdown displays all diagnosis codes configured in your Practice Manager. You can also type letters, numbers, white spaces, and special characters.
Note: You can also type custom information into the “Diagnosis Code” field and save that information alongside the standard diagnosis codes when the field is included in a form.
