From the Navigation Bar, select Forms iQ

‍Navigate to the Practice Form tab and select + New Form

‍In the Select The Form Type dropdown, select Lead Form

‍

‍A New Lead Form with required field will load

‍You can Add and remove Fields to customize your form

‍Add a Form Name

‍Form Description is optional

‍

When you select Save and Preview, you can view the form

‍

Select Submit Lead to save your changes

‍To preview your Sample Lead Form, select Save and Preview

‍‍A window will open showing you a preview of your new lead form
