Understanding Multiple Billable Forms

In Therapy iQ, you have the option to have multiple billable forms.This guide will help you understand multiple billable forms.

1. From the navigation bar, select Forms IQ.

2. Select  + New Forms to create the additional form.

3. Select the  Form Type

4. Input the Form Name

5. Ensure you select the  Default button to set this as the default form that populates when accessing the form from the forms library or calendar documentation icon.

6. Edit as needed and Submit.

The following steps will show you how to select a specific form type for any of your multiple billable forms

7. Navigate to the Forms library under your clients profile.

8.Select the form you would like to view and click the  + New Button to add a new form

9. Once the form is open you will navigate to a drop down that says  Form Type. Select from the drop down.

10. Select which form you would like to use and proceed as normal. Ensure you submit when finished filling out the form.
