From the Navigation Bar, select Client iQ

Search for and select a client from the Client Hub.

Within the client profile, select Forms Library

Navigate to the Addiction Treatment Forms tab.

Select the form type you would like to delete and click on the 3 dots on the right side.

Select Delete.

Select Confirm in the pop-up message to delete the form.
