Once a Practice Form is created, it will be available for all relevant users for documentation purposes. Only users assigned the Practice Forms Permission will be able to create Practice Forms. However, all users can create their own form in Forms iQ.
From the Navigation Bar, select Forms iQ.
In Forms iQ, select Practice Forms.
Select the +New Form button to start a new form.
Select the Form Type from the drop-down menu in the upper left.
Note: Certain billable forms have required fields that will populate in your template, these fields are required for regulation and billing purposes.
Next, add a Name to the form, and add a Description of the form if needed.
Note: You can have multiple of all form types.
Select Save and Preview in the lower right-hand corner to see what pre-populates, and a simple version will appear in a popup.
Select the Field Type, name your field, and determine its width. Click here to learn more about the forms in forms builder
Select the Add New Field button.
Once all fields have been added, Save and Preview your new form.
Select Submit.
The form will now be available in Practice Forms and will be added to the Forms Library
