How to Complete a Toxicology Report

From the Navigation Bar, select Client iQ.

From Client iQ, search for and select a client from the Client Hub

Select Forms Library

Select the Addiction Treatment Forms tab and choose Toxicology Report.

Click + New button to create a new Toxicology Report

Verify Client Name and Date Tested which are pre-populated 

Select ID verification

Record whether Medications are on file.

Record any reported use of drugs or other intoxicants within the last 14 days. If you choose Yes, record Substances, whether it is prescribed, and the Date it was last used.

Select Testing Service Delivered & whether Specimen Collection was observed 

Tick the checkboxes to Select Substances That Were Tested For

Select Specimen Results and record Result Notes.

Select if Specimens were sent to a lab for testing

Collect the Signature, enter the Date, and Submit.
