From the Navigation Bar, select Client iQ.
Search for and select a client from the roster.
From the Client Profile select their Progress Notes.
Then select the + New Note button.
Select Form Type: None then Note Type: SOAP Note.
Enter the Session Details
Note: The client name will automatically be added
Select the applicable Attendance for the session.
Session Attendance Totals will show a count of attended sessions, no-shows, and excused absences
Select the appropriate session Start Time & the Session Duration. The End Time will autofill.
Fill out the SOAP section of the form.
California Users: Toggle the Hide Sensitive Data from the Client Portal switch on or off depending on whether you wish to share that information with your client (the purple switch means the data is hidden)
Ensure the provider signature is filled. The date will autofill with the current date.
Select Save and Submit if completed or Save and Finish Later to save as a draft.
