On the left side of the screen you will find the Navigation Bar, click Client iQ.

Search for or select client from the roster.

Select Forms Library.

Under the Mental Health Forms tab, select Mental Health Biopsychosocial

Click to create a new Mental Health Biopsychosocial form

Complete the form

Select Submit to submit the form

Note: If you click the Save & Close button the form will be saved to Draft status.
