How to Add a Chart Note

The chart note feature is where you can document any non clinical or clinical ( If you choose) interactions with the client, the client’s referral sources, etc. You can use this to document phone calls, emails, face to face interactions etc.

From the Navigation Bar, select Client iQ

Search for your desired on the client list or using the Search Bar

On the Client Profile, Select the Chart Notes tab

Select the + New button to create a new chart note

You will now see a blank Chart Form

The Client’s Name will automatically be generated

If the activity involves a referral source toggle the Does This Activity Involve A Referral Source switch

Select referral source from the menu dropdown

Type in the Contact Person name

Select Activity Type

Select the Date of Activity

Type in your Note

Tip: you can also dictate by selecting the microphone icon

Select Save & Close to finish later or Submit if ready to submit

The Chart Note will now appear in the individual client's chart
