The lead and waitlist manager from the Waitlist iQ Lite can be utilized by your practice to capture leads from your practice website by embedding the provided code.

From the Navigation Bar, select Forms iQ. 

Click on the 3 dots on the right side and then select Edit

This will open your Lead Form

Select the Copy Embed Code button. 

HTML and IFRAME codes are available.

Select Copy Code and then embed it to your site. 

Select Submit
