The lead and waitlist manager can be utilized by your practice to capture leads from your practice website by embedding the provided code.

‍From the Navigation Bar, Navigate to Forms iQ and then select the Practice Forms Tab.

From the Lead Form select the 3 dot icon and select Edit

Select the Copy Embed Code button

The available code will be HTML and IFRAME

Select Copy Code button and embed it on your site.

Note MAC Users: To ensure pasting of the embed code is correct, be sure your keyboard settings are copying text correctly.
